Sign Up & Birthday Promotion (the "Promotion") Rules Lattner Entertainment Lattner Reward App
Lattner Entertainment Group Illinois, LLC ("Lattner Entertainment") Lattner Rewards App (the "App") rewards individuals who patronize our Video Gaming Terminals. We offer the program at participating licensed video gaming locations throughout the State of Illinois. The App is open to all participants regardless of whether the participant has or intends to play a Video Gaming Terminal. By participating in the App, you consent to the following terms and conditions related to Sign Up and Birthday Rewards ("Sign Up & Birthday Rules Lattner Entertainment Lattner Rewards App Rules").
Lattner Entertainment is the sponsor of the Promotion. Lattner Entertainment's actual principal place of business is: 4003 Tyler Dr., Ottawa, IL 61350.
Eligibility Requirements
In order to participate in the Promotion, an individual must be: (a) a legal resident of the United States, and (b) over the age of 21.
The following individuals are ineligible to participate in the App and/or to receive a gift or prize: (a) employees, officers, directors of Lattner Entertainment or Boyd Gaming Corporation, including subsidiaries and affiliated companies and the members of their immediate families (spouses and parents, siblings, children and their spouses and in-laws) and persons living in the same household with such individuals; (b) owners, managers, officers and employees of participating licensed video gaming locations at their own location and/or location where they work; (c) individuals who have entered any self-limited or government agency self-exclusion program or have been otherwise excluded from casinos by a government agency, Lattner Entertainment, or Boyd Gaming or one of its affiliates; and, (d) persons on a Disassociated Patrons, Self Limit, Involuntary Exclusion, Voluntary Exclusion or Self Exclusion List in jurisdictions which Lattner Entertainment or Boyd Gaming Corporation operate, or who have been otherwise excluded from a participating property.
How to Participate
To participate, participants must download the App from the Google PlayStore or Apple AppStore and follow the steps below. The App can be used at all participating locations. Please visit [insert link/web address] for a list of participating locations.
- Download the App on a smartphone device;
- Create an account by providing name, date of birth, e-mail address, phone number, address and create a password;
- Confirm enrollment on subsequent email (participants must login to their email and confirm their account in order to activate).
Participants are automatically entered for the Sign Up and Birthday rewards at participating properties after successfully enrolling in the Lattner Entertainment Lattner Rewards App.
No purchase necessary to enter or participate. A purchase will not affect your chances of receiving a prize.
Participating properties and particular reward programs at locations are subject to change.
How and When Prizes will be Awarded and Notified
A participant will receive a reward after initial sign up and on the birthday entered during the Lattner Rewards sign-up process. Participants will be notified via push notification (if enabled) and within the inbox message of the App. In order to receive a prize notification, participants must have the most current version of the App, location services must be set to “always on” and push notifications must be enabled. Participants will be notified, at no expense to them, within fifteen (15) days of being awarded a prize.
Claiming a Prize
In order to claim a prize, Participants must:
- Have the unclaimed non-expired prize in their inbox;
- Have location services enabled;
- Have Bluetooth services enabled;
- Must be in the beacon area;
- Be at participating property;
- Allow location employee to press REDEEM; and
- Provide a valid government photo ID that matches the participant's information / birthday in the app to claim prize.
- Note: If location services are not set to "always on", when the "Redeem" button is selected, the mobile app will scan for the beacon location manually.
Failure to perform the above steps may result in the loss of the prize. Neither Lattner Entertainment nor location is responsible for loss of prize, nor required to remedy any loss of prize due to failure to comply with the steps above.
Prize Expiration: Participants must claim their prize within 30 days of notification. Failure to claim the prize within the specified time period will result in the loss of prize. Neither Lattner Entertainment nor location is responsible for loss of prize nor required to remedy an expired prize.
Restrictions
- Rewards/prizes are non-transferable and may only be used for the benefit of the individual to whom they are issued.
- Participants are not entitled to exchange or transfer prize or to obtain other substitutes.
- Participants may be required to complete an Affidavit of Eligibility and Liability and a Publicity Release ("Affidavit/Release") before receiving their prize.
- Each individual may enroll for the App only one time (one App Account per person).
- Lattner Entertainment reserves the right to withhold prizes if the Participant does not comply with all requirements in the rules; prizes may be forfeited for noncompliance.
- Individuals will be excluded or removed from the App for providing untruthful, inaccurate or incomplete information, tampering with accounts, tampering/falsifying entries and for sharing use of the same account.
- Individuals will be excluded or removed from the App for tampering with accounts or app/location services/Bluetooth/beacons in order to manipulate the system.
- Lattner Entertainment reserves the right to remove individual accounts for any reason.
- Restricted where prohibited by Law.
Applicable Law(s) Notice / Liability Disclaimers / Limitations
- All aspects of the Lattner Rewards App and the Promotion are subject to Illinois Law and Regulation. Applicable laws include, but are not limited to, the Illinois Prizes and Gifts Act, the Illinois Video Gaming Act and all applicable Illinois Gaming Board rules and policies.
- Lattner Entertainment reserves the right to modify or cancel the App or Promotion to comply with any and all applicable laws.
- Prize recipients are responsible for all costs associated with prizes, including, but not limited to, all federal, state, and local taxes and any other expenses related to a prize. Prize recipients may be required to provide a Driver's License or State ID Card and may be required to complete a prize form prior to receiving a prize. Prize recipients may receive a 1099-A Form if the prize value exceeds a certain/taxable threshold.
- Participating individuals agree to release the Illinois Gaming Board, Lattner Entertainment, Boyd Gaming Corporation, and their respective subsidiaries and affiliated companies' owners, managers, and employees of participating licensed video gaming locations ("Parties") from any and all liability for any claims, costs, injuries, losses, or damages caused by or related in any way to their participation in the Promotion.
- Parties will have no liability and shall be held harmless by Entrants against any liability, for any injuries, losses or damages of any kind.
- Personal Information: Lattner Entertainment and Boyd Gaming, including its subsidiaries and affiliated companies, may use any personal information collected as part of the Program in accordance with App privacy policy.
- Parties are not responsible for incorrect or inaccurate award of bonuses and entries, technical malfunctions, lost information due to failure or interruption of data due to transmission, interruption, or line failures of any telephone network, computer equipment, software or any other error or malfunction. Parties are not responsible for typographical or other errors in the printing of materials, administration of the Promotion or other prizes or awards.
- Lattner Entertainment is not responsible for entries or awards that are not accrued or lost due to service outages or maintenance.
- Lattner Entertainment reserves the right to change or cancel rules or rewards at any time, change any awards and requirements for earning awards, withhold the sign up / birthday bonus for any reason, and cancel a current promotion award at any time and for any reason.
- Rewards or requirements for earning awards may differ by participating location.
- Lattner Entertainment reserves the right to substitute prizes when out of stock or unavailable. Substitute prizes will be of equal or greater value to the original.
- If for any reason the Promotion is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes within or beyond the control of Lattner Entertainment which corrupt or affect the administration, security, fairness, integrity, or proper conduct of the Promotion, Lattner Entertainment reserves the right, in its sole discretion without notice, to cancel, terminate, modify, or suspend the Promotion.
- Any dispute or situation not covered by the above rules will be resolved by Lattner Entertainment management in a manner deemed by them to be the fairest to all concerned, and that decision shall be final and binding for all participants.
